Implementation of Sustainable Social Determinants of Health Programs in Community Pharmacies: Two Possible Models

Social determinants of health (SDoH) account for up to 90% of health outcomes, while medical care accounts for only 10-15%; despite this disparity, only 24% of hospitals and 16% of physician practices screen for the five social needs. Community-embedded and highly accessible, pharmacies are uniquely positioned to connect individuals to local community and social resources and thereby address SDoH.

During this PQA Quality Forum Webinar we will discuss novel community pharmacy practice models that address SDoH, provide real-world examples of these models, and discuss pathways for reimbursement and sustainability. An essential component for program success in all cases has been partnership development and increased interdependence between the pharmacies, local community organizations, and the public health sector.

Presenters from the University at Buffalo School of Pharmacy and Pharmaceutical Sciences will discuss how uniting these collaborators to develop a more permanent sustainability model will be crucial to the continuation of community pharmacy SDoH programs. An expert panel Q&A session regarding the pilot programs and next steps will be held for audience members to engage in active discussion with the SDoH program collaborators from the University of Buffalo and the Community Pharmacy Enhanced Services Networks (CPESN).

This forum builds on the 2020 PQA Leadership Summit and aligns with the 2021 PQA Strategic Plan. During the summit, PQA heard a clear call from members for additional education and recourses on SDoH, which will be addressed during this session and in resources PQA will develop throughout 2021.
