PQA Social Determinants of Health Resource Guide

PQA recently released a PQA Social Determinants of Health (SDOH) Resource Guide Companion, which provides insights on key characteristics of effective partnerships among pharmacies, other health care organizations and community organizations to address SDOH barriers affecting medication access, use and outcomes. The companion guide builds on the third edition of the PQA SDOH Resource Guide. From the first-hand perspectives of individuals leading SDOH programs featured in the guide, the companion discusses types of partnerships in SDOH initiatives and the strategic approaches and supporting program attributes that are common.

The PQA Social Determinants of Health (SDOH) Resource Guide Third Edition documents real-world SDOH services that are promising for improving the quality and safety of medication use. The guide focuses on SDOH screenings, referrals and interventions conducted by or involving pharmacists or pharmacies.

The third edition of this guide profiles 40 SDOH services, including eight new services and updates to ten initiatives from the previous two editions. It covers services that address at least one of seven SDOH barriers and expands the number of interventions focusing on food and nutrition. It also highlights tactics employed by successful SDOH programs.

The guide covers services that address seven SDOH barriers, and many of the initiatives profiled address multiple barriers:

  • Cost of Medications
  • Cultural or Literacy Barriers
  • Decent, Safe and Affordable Housing
  • Food Security
  • Screening for Unmet Needs
  • Social Isolation
  • Transportation

This activity is supported through an independent educational grant from Merck & Co., Inc., Rahway, NJ, USA.

PQA welcomes feedback in the guide and suggestions on SDOH services which can be highlighted in future editions. You can submit feedback and interventions for consideration below.
