Large to mid-size Integrated Delivery Networks (IDNs) that serve much of rural America have seen a tremendous nee d to partner with community organizations to better serve their members.
This PQA Quality Forum Webinar features Sharrel Pinto, the 2021 Community Pharmacy Innovation in Quality individual award winner. Pinto is the Department Head of Allied and Population Health and the Commu nity Practice Innovation Center Director at South Dakota State University.< /p>
Pinto will provide an overview of the challenges facing healthcare or ganizations that serve rural communities and identify statewide collaborati ve solutions that effectively build bridges and break down silos to patient care. Her presentation will shed light on identifying gaps in care within communities and methods for partnering with stakeholders including tribal p artners, federally qualified health centers, and community pharmacies. Fina lly, Pinto will discuss designing specific strategies to working with IDNs and community partners to further improve access to care and positively imp act outcomes.
