PQA Quality Forum Webinar: Immunization Assessment and Documentation by Community Pharmacists Vaccine-preventable diseases can cause long-term morbidity, increased health resource use, and death. As one of the most accessible and frequently visited healthcare team members, community pharmacists are uniquely positioned to improve immunization rates, particularly during medication therapy management (MTM) services. However, there is no standard practice for assessing and documenting immunization status during community pharmacist provided MTM. In this PQA Quality Forum Webinar, Simon Anderegg, Clinical Systems Analyst at DocStation, will join PQA staff to provide insights on an innovative proof of concept to develop the infrastructure needed to capture and standardize pharmacist documentation of patient immunization assessment and compliance with the Advisory Committee on Immunization Practices guidelines. They will discuss how this platform incorporated and examined two draft PQA immunization quality measures. PQA and its members are continually developing new initiatives to inform and close immunization gaps.
