As the health care system continues to evolve with a focus on improved quality at a reduced cost, value-based reimbursement models are emerging in the marketplace to incentivize providers to optimize patient care and outcomes. This presentation describes the collaboration between a payer and a cohort of high performing community pharmacies in Iowa to develop and implement a value-based pharmacy program that incentivizes pharmacists with performance bonus payments by improving patient clinical metrics and total cost of care. Objectives: After attending this Quality Forum Webinar, the participants will be able to:

2. Discuss the metrics of interest within the value-based pharmacy program.
