Value-based care is transforming healthcare by rewarding quality over volume, and pharmacies have a critical role to play. While providers are often the focus, partnerships between health plans and pharmacies can drive meaningful improvements in outcomes for at-risk patients.

Maaria Kheiri, PharmD, Director of Pharmacy Quality Improvement at Centene Pharmacy Services, and Jeff DiGeronimo, BS, Vice President of Program Strategies and Client Success at RxAnte, will showcase two proven value-based pharmacy models: a community pharmacy incentive program and a comprehensive care management program for complex and vulnerable seniors. This is an encore presentation from the 2025 PQA Annual Meeting. PQA Education Director Tammy Malm will host the forum and moderate an audience question-and-answer session at the end of the webinar.
