Quality Forum: Integrating Pharmacists in Value-Based Programs to Improve Performance on PQA Measures

This PQA Quality Forum features Magaly Rodriguez de Bittner, Professor and Associate Dean for Clinical Services and Practice Transformation and Executive Director of the Center for Innovative Pharmacy Solutions at University of Maryland School of Pharmacy.  

PQA measures are used in a variety of settings to improve medication safety, adherence and appropriate use. Rodriguez de Bittner will explain how PQA and other quality measures are used to assess care provided to patients within value-based contracts at the University of Maryland Medical System. She also will share how pharmacist-provided care within the healthcare team can impact measures such as PQA, quality, and health care utilization measures.

PQA Director of Education Amanda Ryan will host the forum and moderate a question and answer session.  

At the completion of this activity, pharmacists will be able to:
• Describe how pharmacists are integrated within a value-based population health program within a large health care system .
• Describe examples of how PQA measures can be integrated into comprehensive value-based care contracts with health plans.
• Discuss examples of how pharmacist-provided care within an interprofessional team impacts select PQA and other clinical measures to improve medication adherence, patient care and reduce unnecessary health care costs.

ACPETo receive credit you must complete an online evaluation and mark down the code that is given for the activity as validation of attending. The evaluation as well as your NABP e-profile ID and birth date MMDD will need to be received by the Washington State Pharmacy Association in order to receive credit. The evaluation must be completed by Thursday, October 14. WSPA will upload the CE within 4-5 weeks after the event. The Washington State Pharmacy Association only has 60 days from the date of the activity to upload the CPE information to the CPE Monitor. It will be your responsibility to check the CPE Monitor before the 60 days to make sure the CE is uploaded in your profile. If there are any problems, you will need to contact WSPA by phone at 425-228-7171. CE cannot be uploaded after the 60 days from the date of the event.
