Collaborating to Implement SDOH Programs   

A pilot program implemented by CPS Solutions, LLC, identified some best practices and recommendations for similar programs. Properly training staff and gaining stakeholder buy-in are crucial steps to successfully designing and launching a program to screen and address health-related social needs.

Highlighting Opportunities for Community Pharmacies   
As pharmacy programs increasingly shift toward value-based care models, there are new opportunities and strategies for community pharmacies to deliver screening, education, and referral interventions in patient care, according to UNC Eshelman School of Pharmacy research. Medication non-adherence results in $100 billion each year in excess hospitalizations, highlighting a crucial role pharmacists can play.

Integrating Specialty Pharmacy to Address SDOH  
recently developed SDOH framework developed by clinicians at Shields Health Solutions demonstrates that health system specialty pharmacies (HSSPs) can have tailored, effective interventions. The framework was piloted at 4 health systems with 50 patients. The workflow involved 5 steps: engage, intervene, coordinate, enhance and renew.

The 2024 PQA Annual Meeting took place in Baltimore, Md., May 14-16, at the Hilton Baltimore Inner Harbor. More than 400 leaders from PQA member organizations attended to network, engage and learn from more than 60 speakers.
